You will not be reimbursed by these programs for sessions. If you're Medicare-eligible you will need to sign a waiver prior to first session.

Payment is due at time of session. Forms of payment include: Cash, Check, Master Card, Visa, American Express or MC/Visa HSA cards. A credit or debit card is required to be on file for auto payment and late cancellations/no shows. If you need to use an alternative form of payment at session please notify me.
